Panel: NASA fails to meet safety test

CAPE CANAVERAL, Fla. (AP) — Despite more than two years of painstaking work, NASA has failed to meet the most important safety recommendations for getting space shuttles flying again following the Columbia tragedy, an oversight panel says.
